Support Coordination
What is the support program?
Support coordination is a program that is medicaid waiver overseen by the New Jersey Department of Developmental Disabilities. It's purpose is to ensure that adults living with developmental disabilities who live in non-licensed setting can access division funded services. These services can include individual/family support, employment/day program, and a plethora of other program. Each individual who is enrolled in Supports Coordination is either assigned or can choose a Support Coordination Agency to assist in this process. Creative Independent Alliance is a DDD approved provider who can be of services to you or your loved one in this transitional process.
